Q1為什麼我常常脹氣、腸胃不舒服?
通常是吃太快、纖維攝取忽多忽少、水喝不夠、壓力累積、外食油脂跟精緻糖過多、久坐少動這幾個日常習慣疊加造成的,很少是單一原因。找出自己符合的項目,先從最容易調整的一兩項開始就好。
Q2腸道菌相是什麼?跟我有什麼關係?
腸道菌相是住在腸道裡數量龐大的細菌所組成的生態系統,依以色列魏茨曼科學研究所團隊的 Sender 等人(2016)的重新計算,數量大約跟人體自身細胞一樣多。這些菌群參與消化、產生代謝物,也跟免疫調節密切相關,而且會隨飲食內容快速改變,是一個動態反應系統,不是一出生就固定不變的設定。
Q3壓力大的時候,為什麼腸胃會先出狀況?
腸道與大腦之間有一條稱為「腸腦軸線」的雙向溝通路徑,透過神經、免疫與代謝物訊號持續互相影響。壓力狀態會透過這條路徑直接影響腸胃蠕動與敏感度,這是有機轉研究支持的真實生理現象。
Q4吃益生菌可以改善情緒或焦慮嗎?
已有小規模人體試驗顯示特定益生菌菌株與焦慮、憂鬱症狀改善有關聯,但研究規模仍有限,機轉細節也仍待更多研究釐清。這類補充品屬於食品範疇,不能取代情緒或精神相關疾病的專業診斷與治療。
Q5「腸漏症」是真的存在的疾病嗎?
「腸道通透性增加」是真實可測量的生理現象,但腸漏症目前並未被主流醫學界正式承認為疾病,也沒有經過驗證的檢驗方法能確診它,包括坊間常見的血液 zonulin 檢測,都已被研究證實有效度爭議。
Q6腸道真的聚集了全身七成免疫細胞嗎?
這是常被過度簡化的說法。依荷蘭烏特勒支大學理論生物學團隊的 Ganusov 與 De Boer(2007)整合多物種測量數據的回顧,整體免疫細胞(淋巴球)並沒有七成在腸道,比較接近 5% 到 20%;但依黏膜免疫學者 Pabst 等人(2008)的回應,如果只看負責分泌抗體的漿細胞,腸道確實佔了全身這類細胞的 7 到 8 成左右,這才是「七成」數字真正站得住腳的地方。
Q7顧好腸道菌相,免疫力就會變好嗎?
腸道菌相與免疫系統的發育、調節有關聯,但多數證據仍屬相關性觀察,「調整菌相直接等於提升免疫力」這種說法過於簡化。均衡飲食與規律作息,仍是目前最有共識的基礎做法。
Q8日常生活可以怎麼調整,幫助腸道更好?
纖維循序漸進增加、每天喝足夠的水、吃飯放慢速度、適度攝取發酵食物、規律運動、留意壓力狀態、三餐時間固定,都是相對容易開始的日常調整。不需要一次全部做到,先挑一兩項最容易調整的開始就好。
Q9睡眠會影響腸道健康嗎?
美國諾瓦東南大學團隊的 Smith 等人(2019)初步的人體觀察性研究顯示,腸道菌相多樣性與睡眠效率有正向關聯(這是該研究中最穩定的一項發現),與總睡眠時間的關聯則只有部分指標達到統計顯著。研究樣本數不大(26 名男性),還無法確定因果方向,但提醒我們腸道保健與睡眠可能互相牽動,並非兩件各自獨立的事。
Q10消化酵素、益生菌、寡糖,需要一起補充嗎?
三者功能不同:消化酵素幫助分解食物、益生菌是補充活菌本身、寡糖則是益生菌的養分來源。可以依照自己的困擾對應選擇,不一定要同時補充。這類補充品屬於日常營養補給,定位是輔助角色,不是藥物,也不能取代醫療診斷。如果日常作息、飲食都還沒調整,通常建議先從這些生活面向著手。
